CANCELLATION AND MISSED NUTRITION COUNSELING APPOINTMENTS
Appointments must be cancelled or rescheduled at least 24 hours before the scheduled appointment time.
If you cancel less than 24 hours before your appointment or do not attend your scheduled appointment, you will be charged the full rate of the scheduled session, subject to applicable law and any limitations applicable to insurance billing.
Late cancellation and no-show fees may not be billable to your insurance company and may be your personal responsibility.
Exceptions may be made at the discretion of Plant Based Nutrition for Life when circumstances reasonably warrant an exception.
Repeated cancellations or missed appointments may result in termination of services.
REQUIRED CONSENT FORMS BEFORE BEGINNING SERVICES
Before beginning nutrition counseling services, clients will be asked to review and complete the following required consent forms through our secure client portal. Documents will be sent to you via your email address.
